[shimV2] refactor scsi controller

• Refactor SCSI controller to directly reference the Controller GUIDs rather than have the controller indexes.
• Introduce StateInvalid in SCSI controller similar to Plan9 controller
• Move test files to the same pattern as other controllers i.e. using mocks
